Colleague 1: Alyssa The National Association of Social Workers (NASW) requires social workers to respect and promote the clients’ rights to self-determination (2021). Social workers should empower clients to identify their own goals and work with them to attain them, aside from a few instances in which the client may be a risk to themselves or others (NASW, 2021). At my field placement agency, a drug and alcohol treatment center, the company’s priority to reduce unnecessary discharges may be misaligned with this value. Discharges that are considered against medical advice, may occur when an individual wishes to leave the facility at some point prior to the standard length of treatment. These discharges often happen if a client experiences a loss outside of treatment, is concerned about their job/children/family, or just may not feel ready to follow through with treatment. Banyan’s mission is to reduce the number of against medical advice discharges at whatever cost. The company encourages therapists, group facilitators, and behavioral health techs to spend several hours attempting to persuade a client into remaining in treatment. This is often an emotional and time consuming process. While, encouraging clients to remain in the facility and complete treatment may seem like the goal for the clinical side, clients have many reasons for electing not to stay at that time. Some of the strategies or processes I’ve observed to convince a client to stay in treatment, may slightly infringe on the client’s right to self-determination and empowerment. My field experience, Pyramid Healthcare, Inc., an inpatient drug and alcohol facility has many organizational policies. One policy that is imperative to client service is ensuring all consents and releases are signed completely. This is something that gets completed during intake and is explained to them in full what each release and consent form is for. There are five different release forms each individual signs, two professional releases and three restricted ones. The two professional releases are for the local medical and crisis center, in the event of someone needing emergency medical and psychiatric attention, the individual, as well as Pyramid, will be covered and protected. The three restricted release forms are for funding, each release states the primary funding and two backup funders to ensure that no matter what, an individual is covered for their treatment. This policy is in place to ensure the individual has access to treatment and emergency services as needed. NASW Code of Ethics stated that social workers should use clear and understandable language to inform clients of the purpose of services, including informing them about the consents and releases as follows. Organizational policy is a set of rules developed by the agency that sets standards for how that agency operates. In addition to formal policies, agencies also follow specific informal policies as well (Birkenmaier & Berg-Weger, 2018). These informal policies can create dissonance between a social worker’s ethical obligation to their work and the practical demands of staying financially alive (Gallina, 2010). A study of over three hundred social workers revealed that most feel an ethical conflict between their professional ethics their agency’s organizational policy. An example of this was terminating clients who could no longer pay for services, even though they still needed those services (Gallina, 2010). In my practicum site, I recently started treating a young man who had an eating behavior that his grandmother wanted to get under control. Due to the family having Medicaid, I was able to schedule him for a follow up appointment right away. If the family had private insurance, however, I would have had to place him in an orientation therapy group until his insurance cleared him for services. This definitely creates an ethical issue when a client really needs services but, because of their ability to pay, is forced to wait in a therapy group that might be little or no benefit to them, while they wait for their insurance to go through.
